Remove unused code in caldav component node

Fix broken MKCALENDAR code - always failed if a body was supplied

Further fixes to proxy handling of collections that require cloning, e.g attendees. Allowed us to handle attendees like other collections.

Remove method parameter from implicitSchedule - not used.

Better handling of replies - we were replying to the sender

Some fixes to creation of master for detached instance. Set dtstart to same timezone as instance. May still have problem with floating recurring events.

Fix setting transparency in auto-respond
